NOTICE TO PATIENTS: GOOD FAITH ESTIMATE
Under the law, healthcare providers are required to give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical services.
At our chiropractic clinic, you have the right to receive a Good Faith Estimate explaining how much your care will cost.
What is a Good Faith Estimate?
A Good Faith Estimate provides an expected total cost of any non-emergency services, including exams, adjustments, therapies, and any other recommended treatments. This estimate is based on the information known at the time it is created.
Who can receive a Good Faith Estimate?
You are entitled to a Good Faith Estimate if you:
- Do not have health insurance, or
- Choose not to use your health insurance for your care
What should you expect?
- You can request a Good Faith Estimate before scheduling a service or at any time during your care.
- If you schedule services at least 3 business days in advance, we will provide you with an estimate in writing.
- The estimate will include expected charges for the primary services as well as any related items or services.
Your rights:
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
- You may save a copy or take a photo of your Good Faith Estimate for your records.
